| There are two different types of CORI forms. If you live in Massachusetts, fill out the MA CORI form. If you live outside of Massachusetts, fill out the Out-of-state CORI form.
If you live in Massachusetts but attend school out-of-state, OR
If you live outside of Massachusetts but attend school in MA, fill out both CORI forms.
